Grantees are required to to notify the Foundation at any time if there are major changes in the organization, such as new staff, program adjustments, etc.
A grant year-end report outlining the program's successes and challenges will be required as well as a financial accounting. This report is submitted online and grantees will need to use the same log-in information linked to your application in order to access the grant report. An automatic email will be sent to the primary contact on file when the online reporting link is activated, approximately one month prior to the due date as outlined in the award notification letter.

When a multi-year grant is approved, payment of subsequent installments is made conditional on receipt of the above report and upon review of the Trustees.
Grantees must take an application cycle off after submitting their final report before being eligible to submit a new request.
